my form is here. it wasn't linked on the thread due to a timezone issue, but Chimi allowed me to send it to them through PM. ;w;

if it seems a little bare-bones, well, that's because I made an rpg game to go along with it that helps to explain my concept and tell a story, as well. it pretty much contained most of my ideas, haha. while it was technically "finished," as in the basic story was there and the game could be beaten, I think I'd like to work on it more and expand the adventure before releasing it to the public. I've got a much bigger story in mind, and I'm just so glad I'll be able to keep working on it. ;o;
