I am sorry, I was on mobile when I posted the form and it was rushed due to me having a lot of other things to do.
I can edit my form so that is anyone sees it after they know where it came from.
I do apologize, it was a rushed form and I randomly grabbed the pic of google search.

Also, when I found it, it brought me to this site so I did not see or get any rules.
