Remembrance was galloping alone on the shore when he saw a young girl struggling in the frothy waves. Feeling pity for her he leapt in and swam to her. Remembrance then took her back to shore and made sure the girl was all right. After taking the little girl back to her parents, she gave him a kiss. The mark has stayed with him ever since. <3

Okay, how he got his heart shaped spot.
One day, his owner was grooming him. She was doing a very gentle brush, it was like a back scratch.
Then, she packed up the brush an put him away. Since the back scratch made him so relaxed, he fell. When he fell, he got water on him in a heart shape. Whe. The water dried, it left a white heart on him, the end!
