Yep, Kai pretty much nailed it, you need a form, but it's not based on the design. It's based on the form and the story..^.^

The form is located on the original post where you entered, and if there is no form added by the time it ends I'm afraid it won't count as an entry... It's a good design, though you might want to fix up the edges a little
