His priest outfit is mainly made of salvaged materials that Kain has put together for him. Kain might not understand his husband's religion or priesthood/godhood, but he's very supportive.
The nose ring has a heart and Kain's name engraved on the inside of it, so, no matter how long Calchas is gone on god duty, he has a part of Kain with him.
