Hi Jenna! Nice to see a new player! So, I am the owner of the parents of this YeYu. When I first started out, I also was confused. So, to fill out the form, all you do is this:
Name; Ye Ye
Lets say that I think that Ye Ye is a good name, I'll name it Ye Ye.
Gender; Female
I feel that it is a female
Up to 15 word personality; Up to you
This is where it gets tricky. With personalities, you just have to be as creative as you can, and abide by the rules. For this YeYu, you are allowed up to 15 words for the personality. Hope you get the guide!

On how to adopt, well, just fill out the form (if there is one) and hope that you win! No, this will not appear in your account, just so you know.
