If Dio could change one thing about the world, he would eliminate illness. Illness is the cause of endless amounts of suffering in the world, suffering that can be ended with the removal of it. Eliminating illness would increase the lifespan of the world's organisms, but it would not stop death from occurring.

"I know I cannot stop death from occurring with this wish, but I can prolong protection from its ghastly arrival. I have lost many to illness, and the pain is unbearable. So many unforged memories left in my mind... so many experiences never come to light...
