it can be either one, but there must be only one drawing in your form.

ehh, only as many as you need to show the details. i'd say at most two would be fine to show the front and the back, unless there's something else you feel the need to add too. like if you put a design on the inner collar, for example.

reasons behind collar . when Chianti was five years old, her mother died of a extremely deathly illness. it was very tragic for Chianti because her and her mother shared a very special relationship. her mother had a love for bandanas, and she had colors all throughout the rainbow. Chianti customized her bandana to her mothers favorite colors. she added dots and stripes as well because her mother had an obsession with dots and stripes. she included the date her mother died so that she never forgets. Chianti rarely takes her bandana off, even at night. on the inside of the bandana, it writes "I love you... forever." it's very emotional for her and usually doesn't share this with everyone. wearing it makes her feel close to her mother again.
