well nothing happens if you spare him, but there are two different endings you can get.
(i can put these in transparent later if you'd like.)
if you DO NOT hit mettaton neo with full power, thus saying, hit the end of the radar, you get the queen alphys ending.
if you abort the genocide route in hotland, i think you get the king mettaton ending.
