Congrats on your twins. The top is a normal baby.
The bottom baby was chosen to become a ghoul sima - a.k.a, ghoul simas ca be chosen outside of births with ghouls in them. So its possible to get a ghoul from normal simas, just not as common.
Whe this baby stop growing, whether you pps them or not, after 3 weeks they may request to transcend to reaper status. This will give them transparency and they will be "dead" - but a reaper.
