fantasy and medieval are really really good
maybe the landscape could be a mixture? so like a rainforest that gives way to a American/European woodland and then meadows and mountains with lakes?

there could be a castle with a city

maybe it could be near the mountains, and two sides flanked by meadows (one with a lake?), one by the mountains and the fourth by a woodland?
then there could be settlements literally everywhere

but we still need a plot; anyone have any ideas?

Maybe famine and disease amongst the commoners would lead to civil war or something like that? Or maybe there could be some ancient prophecy that foretells the awakening of some sort of evil?

I don't know, those are just my ideas.
