Sorry for taking this long! Here are my forms for a DuskClan apprentice and DawnClan Med. Cat.
I may also add a DawnClan elder soon, but I decided that it's more important to have these two up first.

ＴＲＩＰ ＴＯ ＴＨＥ ＰＡＳＴ

Poppyfrost had been born and raised in DawnClan. Her mother, a stubborn and strong-willed she-cat, had gone into labor during a Gathering. Only the Medicine Cat Apprentice was available to help with the birth, and being a very new apprentice, he had little knowledge of what to do. Poppyfrost and her sister, Jasminetail, survived, but her mother and two brothers were lost. Throughout her kithood, she had wanted to be the Medicine Cat and prevent anything like her birth from happening again. Only two days after becoming Lilypond's apprentice, Poppyfrost's father drowned in a creek while searching for a rogue fox. He jumped in to save an apprentice, who had been knocked in by the fox, but both were killed. Despite the deaths of her father and friend, Poppyfrost was all the more eager to learn. She became the Medicine Cat only 9 moons later, after her mentor's sudden death. The stress of her teacher's death hung over her like a dark cloud, and she was silent and depressed for a moon straight. The only thing that snapped her out of her trance was the death of her sister. Jasminetail's infected wound had been left untreated for weeks, and the infection became fatal. Poppyfrost realized that, if she was to do her job, grief and solitude would only hurt her Clan. After a life of death and distress, the she-cat was finally ready to do good.

ＤＩＧＧＩＮＧ ＤＥＥＰＥＲ

Poppyfrost has always tried to be optimistic, but hasn't always succeeded. Her shy manor caused her to be outcast as a kit, since she had trouble making friends. Her closest companion was her energetic sister, Jasminefrost, who encouraged her throughout her life. From an early age, Poppyfrost was gentle and selfless, although she became more self-involved after Lilypond's death. She had always been caring and forgiving, even to her worst enemies, but don't put it past her to hold a grudge!

ＴＲＩＰ ＴＯ ＴＨＥ ＰＡＳＴ

It was a crisp winter morning, the day Brookepaw was born. Her mother was a weak warrior, and her father was a loner with no feelings for his 'mate'. Brookepaw's devastated mother climbed a maple and 'slipped' only days after the birth of her kits. With no other queens to care for Brookepaw and her brother, the duty fell to the DawnClan medicine cat. However, some cats in the Clan reacted badly to half-Clan kits. Brookekit and Galekit left, at only 4 moons of age, and went to live in DuskClan. Two moons later, on the night before their apprentice ceremony, Galekit disappeared. Brookepaw still dreams about living among the trees, feeling uncomfortable in the open moors. Yet she's made friends in DuskClan, and wouldn't leave them for the world.

ＤＩＧＧＩＮＧ ＤＥＥＰＥＲ

Some of the other cats call Brookepaw "Brookekit," simply because she acts like a kit all the time. If you ever find her sitting still, you're witnessing a miracle. She's incredibly excitable and curious, always wanting to do what the warriors do. Although she's very fast, Brookepaw is too impatient to be a good hunter. She excels in combat, working for moons on a single battle move or strategy until she's got it down perfectly. Rarely thinking things through, words and ideas simply slip out of Brookepaw's mouth, and this makes her rather obnoxious and reckless. Although she doesn't always show it, her heart is in the right place.

ＴＲＩＰ ＴＯ ＴＨＥ ＰＡＳＴ

Not much is known about Patchtail's history, since he shuts right up when anyone asks. He was one of three kits, a brother named Scratchside and a sister named Ivytendril. Ivytendril was killed on her first patrol, when her group was ambushed by a badger. Scratchside and Patchtail never saw eye to eye after their sister's death. Patchtail even gave his brother the scar that would cause his name-change later on. Just as Patchtail was becoming an experienced warrior, his entire family contracted greencough. Patchtail survived with little damage, but his family was killed swiftly. He mated Willowflight, and though she never managed to carry kits full-term, the unlikely couple shared undying love until her death by poison berries. Soon after, the warrior decided that his combat life was over. He retired to the elder's den and settled in for a long life of grumpiness and solitude.

ＤＩＧＧＩＮＧ ＤＥＥＰＥＲ

Patchtail has always been the quiet, brooding type. The only emotions he ever showed were annoyance and huffiness. Preferring to be left alone, he had no friends and many enemies, though he was prized for his skill in hunting birds. Patchtail's favorite freshkill was and still is the thrush. Often, he'll bribe the kits and apprentices with stories, in return for a nice thrush or robin. That's the only way anyone can get more than two words out of him. Since he's used to living alone in the elder's den, Patchtail tends to not socialize or go to Highrock when called. He'd be content to just be left alone to die, but his health seems perfect and he won't go to any lengths to stop that. Although he's cold and anti-social, he has a soft spot for a certain cat that reminds him of someone.

Larkstar couldn't quit wondering about the murderer in the Clan. Deciding to go ask for advice, she went to the Medicine den.
"Poppyfrost, may I have a word?" Like most leaders, Larkstar was friends with her medicine cat. She still missed Lilypond, but Poppyfrost was an amazing she-cat, and had gone through a lot. Larkstar wondered how she'd managed to stay positive after all of the tragedy she'd seen.

ᵛᵒˡᵉᵖᵃʷ Volepaw walked out of the elder's den with a shudder. The apprentices had to do all the nasty jobs, and he wouldn't mind so much if the other apprentices at least helped him out. He cleaned his pelt thoroughly before going to the nursery to see if the queens needed anything.

ᶤᵛʸᵖᵃʷIvypaw dug her claws furiously into the ground after a rabbit escaped them. She hissed quietly, trying to find any other prey. She'd caught a mouse, but it was small. At least it would feed Wolfkit, the only kit in the nursery. Ivypaw sank to the ground, her shoulders slumped. Misery made her sigh. Ebonypaw seemed okay, but he hadn't spoken to her since before the vigil started the morning before. Warmheart was gone, she was like Ivypaw's kin. She'd helped take care of Ivy"kit" when her real mother hadn't. Now Ebonypaw had changed inside.

Poppyfrost's head jerked up in surprise, then bowed back down when she noticed Larkstar. "Hello. A word about what?" she added, a bit surprised at her leader's request. They had always been friends, but lately Larkstar had seemed a bit distant, or lost in thought.
