Probably because in The Nutcracker, Clara is at a party.
If this pet is supposed to represent The Nutcracker, or Clara, or even a dancer in the show, the party balloon would represent party scene. c:
It could also be the kissing doll. c:

I'm probably off with all this, but that's what I've always thought.
Also I'm a dancer myself, so that's what comes to mind. ;P

There are also three piercings on it's nose, one on it's lip, one above it's eye and I think there are some on it's fin.

There's two piercings above it's eye, a smaller one on the top right of the bottom one. (: I see the piercing on his mouth as a tooth/fang hanging out.

There's three on his nose like you said, and there's two more on the other side of his nose too!
