Perhaps the biker that pushed him away was (as we all believe) was used to distract him from Sherlock's body so the seemingly normal crowd members could make a believable death scene. Was this the favor from Molly?

Sherlock jumped, landed in the truck, and drove off safely.
The actors, volunteers, whoever they were, rolled the body out with a mask/cask of Sherlock's face. It didn't have to be perfect, John was disoriented after the man hit him with a bicycle.
When "Sherlock" is wheeled off into the hospital, the body is returned to the morgue and the trick is done.
