I think the idea of adopting out accessories that morph into animals is rather unique! I have seen a few things with a similar theme, but nothing directly like this one c': For the forms you should require the applicant to write out why they would want it, how it would be used, etc. You might also want to require that they draw their character wearing the accessory so you can see how it fits :'D
