You can place one request per opening. No more. You may not send payment to reserve a future slot anymore either sorry.

Everything cost 5 event tokens.
▒▒▒ On The Subject Of Modifications▒▒▒
You may only order modifications that are listed on the modifications list. No exceptions. This means, you may not request glasses, bracelets, hair, and so on.

Once you have read all of the rules and sent payment, go ahead and submit the following form. I have created a generic form to allow you to reformat according to the template you would like :3 Please be specific, you should tell what color you want for each part of the costume or I will decide for you. For example, you might say you would like the vampire Cape to be dark blue on the out side and Apple green on the inside with white teeth tipped in blood. Please do not just say you would like the costume to be "blue" Dx
