So this one should be easier to get.
I had a lot of fun doing this one, so I want you to have fun getting it! Make your own form.
You Form MUST Show:

-A name
-A gender
-That you will care for and do art of this Muusika
-That you acctually WANT it and are having a good time

You must also include a symbol in your form that reminds you of this Muusika; this is, after all, their favorite symbol. This is an opinion thing, though, so you may want to explain why you picked that symbol.
You can incorporate these things into your post in your own way; it does not have to be a form. For example, it could be a skit, a poem, a comic, whatever. As long as you enjoy it.
