Actually, I've been stalking your form. I really like it because it stands out to me and it looks like you put a lot of effort into it.

I love it <3

The only critique I have is to maybe put a space between the large title words and the smaller text. Also change the purple letters to a color that suites the design more, perhaps a dark red or aqua. Unless you specifically want purple. ^u^
