mine is a little differnt and I have it typed up so im not sure how to fill the form out with the way I have it sorry...
Description:
Dragon-
wings: feathered or feathery ending in wind-like wisps
body: sleek and strong with feathery ears, slightly featherd jaws and back of legs
colors and design; possible colors: grays, silver, white, lt. blue, gray blue, etc. design: maybe some clouds, wispy markings, a tornado looking design?
Background: Only needed to accentuate features (in my opionion)
Views: if possible, One front and one side

Just wondering, I know the shop is currently closed but on the first post you mention humanoid creatures, does that include humans as well? Just wondering for if I do choose to order once the shop is open again. ^^
