You don't need another form for that, you don't even need the images. The price is one pair of purple socks + one pair of candycorn socks, if that's what you meant your form is fine^^.

oh, i applied on the last page with the white ish dog and then with the brown tree dog. if i do get accepted you may choose eather one or take both if you wish
