I really like the detail you went I to with the personality, it seems to work with him well.
With the form itself, I quite like it but make sure it isn't too bright, maybe the titles could be a darker blue?

I like your story but i'm going to be a bit 'nitpicky'.
It seems slightly un-realistic that he would smash the glass with his head, maybe having the door thrown open by the crash?
Also does your character feel the pain of her wounds, I think some more reaction by her would be nice.

Other than that just double-check spelling and I think your form is really well done!
