When people have similar usernames to people I know.
It always confuses the crap out of me when they post, especially when the two are only separated by an extra period or a space. It seems like they're trying to copy them to be more popular or something.

My teachers don't even bother saying anything to me anymore when I doodle on papers, no matter how deformed or odd the creature I'm drawing is. They'll stop calling you out on it eventually.
