I don't think there's a ready made form, but you just need basics like name, age, gender, species, game (if in one), personality, etc.

Okay guys I'm back~ it seems like everyone has stuff planned and I don't really want to disrupt any of that, so is there somewhere it'd work well for Easifa to go? She's still in Hyrule Castle Town right now but with everything going on there I can easily justify her running off.
