I've never personally used one, but I do know people have had some success with them. It really depends on the cat, some cats might not be as interested as others. If your cat is easily startled or takes a while getting used to new things, you might have some trouble getting them accustomed to a cat wheel. This video might help you figure out how to build it, and there's this Pinterest page with some other examples of DIY cat wheels.
They're also, usually, not very quiet. It's a big wheel with a few-pound cat on it, it's bound to make some kind of noise. If you don't want to listen to the noise all the time, I'd suggest building it in a room that you can easily block the cat from going into when you don't want them to be on it.
