I don't think there's a template form, per se, you can just make one. )

If it isn't too much trouble, is anyone willing to help me a bit with the template of the form? Nothing fancy or anything, I just want a workable form lol

I mean, you can take the format of mine if you'd like, just copy the coding and replace all the details and quotes. My forms are really long tho so idk if you'd prefer something shorter. If you don't mind, however, you can take my form if you just replace all the content HAHA
