there isn't a set form, you can use your own, but heres one i made a while back for people to use. its quite simple.

Personality;
Optimistic - usually hopeful and confident about the future, but in dire situations his optimism can quickly turn to pessimism.
Curious- always eager to learn more, though he can go a bit to far at times and get himself into trouble.
Extroverted - outgoing and socially confident, not afraid to speak his mind when he feels he needs to.
Cocky - conceited or arrogant at times, typically because he wants to be noticed.

History;
Nothing much has happened in his young life, he was born an only kit and he likes it that way. However, no siblings can also be lonely at times, and when he tried to make friends, he was typically ignored. Molepaw has yet to see battle, but he has been training and seems to look forward to it. From time to time, he does tend to wander off in hope of a catch that will impress his clan, but after a few good scoldings the likelihood of Molepaw's little "adventures" continuing are unlikely.

you can just shove him into the roleplay right now. theres a lot of confusing chaos going on but everyone will pretend like he was there the whole time.
