Idk. I sent mine in during the period of time the mod application was taken down. (Arty admitted it was an accident and should've still been up) He PM'd me the form after that so I just PM'd my application in the reply to it.

How active are you?: i am online everyday. on weekdays, i am usually on for around 1-3 hours, it depends on how much homework i have that day. on weekends, i am online for 4-6 hours [with breaks in between of course], but i may have some special events/occasions on weekends, so those times will vary.
Will you promise to follow the rules and inform others to follow them too?: yes, of course.
Will you pm me if the thread is dying or if something is wrong?: if the thread was dying, i would try and host a game and see if i could get it back up and running, but if it were something more serious, as everyone lost interest in the game totally [which i hope never happens], i would pm you.
How would you respond if someone is spamming?: i would kindly tell them to stop. if they continued to spam, i would pm you about the issue.
How would you respond if someone was insulting someone else?: if someone was insulting someone else, i would give them a warning about being rude to other users. if they continue to insult member/s i would take it to you to decide their punishment.
Why do you want to be a mod?: i really enjoy these hunger game sims-- i even have a discord dedicated to them, my life is sad-- and i would love to host these rounds. these games bring people together, and the mods help do that. i want to help people come together,
and make new friendships through this game. as a mod, i would not only do my duty, but i would also make sure to add a bit of fun to the rounds, to keep it happy <3 i would love to be a mod, and i hope you consider my application. thank you, mochi.

How would you respond if someone was insulting someone else?: I'd kindly tell them to stop if it's not that bad, then give a warning if it persists and then report if things get worse.
Why do you want to be a mod?: I want to be able to host rounds for others and help keep this as active as possible.

Don't get me wrong!! I'm ecstatic Wolfpaw won on my very first round, and Pipit so soon after.
