if a leader doesnt beleive in starclan, do they get their nine lives?
no.
imagine a clan this happened to.
"Namestar just lost a life."
"Then why isn't s/he waking up?!"
"Namestar?"
"NAMESTAR?!"
;-;
Isn't that what kind of happened to Nightstar? Runningnose just told the Clan he lost all of his lives through the sickness.
(spoilers) No. Starclan refused to give Nightstar his nine lives because Tigerstar was still alive. He did believe in Starclan, and he did go to the Moonpool. Starclan just rejcted him.

Warriors WIki wrote:Fireheart learns from Runningnose, ShadowClan's medicine cat, that Nightstar had not been granted nine lives by StarClan because Brokentail, their old leader, was still alive and StarClan still recognized him as leader. By the time ShadowClan had learned of Brokentail's death, Nightstar was too weak to make the journey to the Moonstone to receive his lives. Because ShadowClan would have panicked if they'd known the truth, StarClan's rejection of Nightstar was kept secret and the Clan cats were convinced that the sickness had taken all of Nightstar's lives at once. Runningnose believed that he was not the right leader for ShadowClan.
