That actually is something that is possible.
It means that the splinter has a sudden change in physics, just at the moment of explosion.
Or, it causes a set of events to lead up to the explosion that WOULD be possible, depending on the exact circumstances leading TO the climax event.

But what if they imagined their head exploding for absolutely no reason and therefore it would have to explode for absolutely no reason

The physics in that universe would change suddenly to cause the explosion to happen as imagined, and then the universe will be cut off, for having been finished.

if a story in a universe could end at something as simple as a coinflip, how do they know when to cut it off

Cutters have methods of measuring theses things.
It's part of their job.

what if someone created a story where things were supposed to happen after the end and after the story ends and they live happily ever after, like a neverending story

It would continue on until the point that there is no conceived continuation.
So if there's a sequel, it continues in the sequel until such point that there is no concept of continuation.

but what if the main part of the story is the fact that it never ends
like I'm writing a story where at the end one of the big concepts is that Time goes on forever and ever and ever

It would be like its own self contained loop.
