Upper right-hand corner of the post is this little thing called "edit". Go to the post with the form, delete the second. Put "NVM" or something, or else it won't be submitted. Alternatively, report it, ask for it to be deleted.

Use the edit button instead of double/triple/quadruple posting. It's considered spam and makes the mods angry.
