Hmm... You appear to have filled out the first breeding form like you would for the second breeding form.

If you want to fix it, do you see on the first page how Salki filled out her breeding form? That's the correct way. I can work with how you have it now, but the marking order on the pups would be random as opposed to how you want it, which is the advantage of filling out the first form.

Erm... I dunno if that made sense to you or not. If it didn't, just tell me and I'll try to explain it another way.

Hmm... Just about ^.^
You can't unfourtunately pick one or the other marking like you did with Swirls and Spiral. Just move those two onto their own separate lines.
And since only one of the parents has Front Half, if it shows up on a pup, it'll be 3d2518 regardless of the base colour. If you could correct those two things, then it should be filled out right ^.^

EDIT: All the markings of the parents have to be in the breeding form, which includes Front Half. I know the halves are a kind of double edged sword. They're great for hiding part of a marking, but if they show up in a breeding they can make a pup look odd XD Of course, you can always breed them again if you don't like the way the pups turned out ^.^
