Okay, a few pointers on your form. What you have for the background/history section should go in the use section where you have the "?". The use section is where you put why you want the pet and what you'll do with it.

The background/history section is for what her life was like before she met you or what her life was like as a pup, just basically describing her life. I'm sure you can put a nice form together. If you go on the front page, you'll see some examples of a winning form. <3

Yes, you have the idea. It's getting better. Now try to make it longer. Desribe the place where you met her. Talk about her being so thin that her bones were showing a bit. Talk about the delicious food the you promised to feed her and how you gained her trust! ^^

The code drop, that's when an adoption agency decides who gets the animal, is not until next Saturday. You'll have to wait until then to see if you got her. But anyways, please add more to your form. Give the animal a history and a personality, even if it is short.
