You can ask for a specific variant for your custom, so you'd only need the normal request form.
However, you could ask for a non-lineart change custom and use Surgical Tools to get small edits.

Proposed theme: A black heart corrupted by gold. This tracker was designed specifically to carry out the tasks no human could stomach, essentially intended as an executioner. Slowly, the dark and dangerous nature of this particular dog has become corrupted by an undeniable sense of loyalty, of servitude, but not to their creators. In a desperate search for an unattainable goal, this creature became feral and abandoned post in search of belonging. With an affinity for crystals, mushrooms, and old bones, they keep their journey to forests and caves, wandering the world in hopes of finding that missing piece, but afraid of what they might do to that missing piece if they ever find it.

I look forward to seeing what you come up with! I don't have any particular specifications other than the main colors must be black and gold.
