As sad as I am to see that Arrow needs a new owner, it really couldn't be more convenient.
See, I still owe Wolf_Bringer_1999 a runner up for Arrow. Now that he's being put up for re-adoption, I'll see if they want him instead of a runner up, since they wanted him all along.

So instead of someone else PMing you, I'll ask if Wolf_Bringer_1999 wants Arrow. ^^

I like the form idea.
I was wondering if you want it to be like a contest thing, with it's own Oeakai page. Where everyone can enter if they'd like?
Or do you want people to PM/email/whatever you and get them to fill out a form like that? ^^
