Some people that get surgery and hrt (hormone replacement treatment) identify as transsexual, but that term is mostly outdated and describes those specific people in the community, not a lot of people identify as 'transsexual' anymore. Transgender is an umbrella term that just describes a person who identifies as a gender other than the one associated with the sex they were assigned at birth.
