I've actually never changed any of my characters designs. X3
I often add accessories, but I don't change them. ^^

How often do you change the design/accessories of your characters?
Hrm. Well most of mine I never change, just cuz I'm lazy. /shot
One of my characters, Aleia, that I have the most art of doesn't change for a reason, and that being because I'm lazy again I believe his accessories and design fit him too well to ever change.
Another character that I use, Auggie, is always changing his accessories/clothes, though his design stays relatively the same way.
