i actually don't think that mike is the phone guy- mike is the character that you're playing the phone guy referring to the person who calls you at the beginning of each night (obviously)

Why did they never show a photo of the phone guy, or said his name?

They might not have shown him because he was the one in the Bite of 87, and they don't want his name out because people will ship him mike might remember his name as the kid getting bit ( it was probably on the news, the news says names ) so he hides his identity.
