Famine and Pestilence are usually pretty closely associated, so if you want to combine hunger and sickness it'd get 'em both down in one go 8D

Yulynh, the artist that made the set of apocalypse horses included Pestilence as a substitute for the anti-christ conqueror type so as to avoid the religious side of the horsemen, so pestilence was kinda-sorta made up, but could probably be mushed together with famine. But really, this sounds sosososo epic I may have to get up and do a jig
