Number One
What I'm offering is a full reference sheet for your character. What the base payment (see number three) will get you is:
-A full body, flat colored picture of your character.
-A simple backdrop. (A solid color with a few simple designs here and there)
-Any amount of text (as long as I can make it fit and still have it be readable) about your character such as name, gender, personality, etc. on the reference sheet.
-The color splotches.
-Four 'extras' drawn separately from the main drawing. (One extra could be a different outfit or a necklace that the character always wears)
-For animal characters, there will be an open mouth on the reference sheet where the colors in the mouth can be displayed.
-For characters with a detailed designs in their eyes, I will draw bigger eyes on the reference sheet so you can see the designs.
-For characters with tattoos I will draw a bigger version of the tattoo on the reference sheet.
-For animal characters, there will be paw prints on the reference sheet with the paw pad colors.

Human characters will be drawn facing forward and animal characters will be drawn on a side view. For animals with asymmetrical designs it won't cost any extra to have it drawn from the left side and the right. If you want your characters drawn at additional angles (For example if you want a human facing backwards or if you want a dog facing front ways) you will have to pay more.
Any additional 'extras' besides the four I'm already giving you will cost extra.
A backdrop with more 'complex' designs will cost extra.
Number Two
My first example is of my character, Jin:

This background would be an example of the more 'complex' backdrop. I know it's not much, but they actually take a while to make.
Everything else in this reference would have been covered in base payment, his four 'extras', the text, the head on, fully body drawing of him, and the text.
My second example is of my fursona, Sunny:

Everything in here would be covered by base payment.
You'll notice that the background is made up of circles, that I will not do. Instead I would make it a solid block like you saw in the previous example, but the designs within the circle, the thing that looks like a wheel, would be an example of the 'simple' background that's covered in base payment.
Number Three
The base price for the reference sheet will be three SEPTEMBER '08 DOGS.
This price is non-negotiable unless I can be paid in multiple Pink Shima Butterfly Wolves or something higher than the UR Horse on the "Rares List".
-For up to three additional accessories add another 08 September dog
-For a more complex background add another 08 September dog
-For additional angles of your character add three more 08 September dogs
-For a more complex background add another 08 September dog
-For additional angles of your character add three more 08 September dogs
Once your form has been accepted you have to send a trade which I'll accept immediately. Normally I don't accept trades unless I've already finished the artwork, but these pets will be going to Akami-chan's and I's contest so I'll need the pets immediately. I will get done the references as soon as I possibly can, but please be patient with me because I have a lot of other projects to deal with, not to mention personal problems and school coming up very soon. ):
You may of course ask for updates on how your reference is coming along and ask if I have an estimated time for when it will be finish, but please do not ask me in a condescending manner because I tend to get snappy with people who come across like that.
Number Four
How I want you to fill out the form. I'll fill out a form as an example and explain further each section.
Payment: -Link to GROUP WITH PETS-
Please do not tell me 3 '08 Septembers. I already know what I'm charging, I want to see EXACTLY what you're paying and proof of payment. You can link to each pet if you'd like, but I'd prefer you to make a group for them. Also, do not send a trade until I say I've accepted your form.
Character Name: Coffee Girl
Self explanatory. You can tell me if you don't want me to put the name of your character on the reference sheet for whatever reason, and I'll leave it out.
Character Description: -Insert long detailed description of character here-
Instead of actually writing it out, I'll just explain. Do not tell me 'A spotted dog' or you will most likely be rejected, here's what I need on this part of the form:
-Any markings/tattoos, their size, color, and placement.
-Colors used on the character, including colors of accessories, paw pad colors, mouth colors, hair/fur colors. Anything. (If you could make a little page on MS Paint or something with all the color splotches and upload it to Tinypic or Photobucket I would appreciate it, otherwise I won't know the exact color you want.)
-Anything essential to their design that you want in the reference. (Piercings, clothing, a teddy bear, whatever. As long as it's covered in the base payment or you are paying extra for it, list it.)
Pretty much I want the most detailed description possible that you can give me. If you have left something out of your description and fail to tell me until after you've seen the completed picture, I will most likely not fix it unless it is an easy fix. So try to put absolutely everything you can into this description.
Picture of Character: -Link to drawing/old reference/any picture of this character-
This, of course, is not mandatory. If you have no art of this character simply put 'none'. If you have any doodles you've done of this character or another artist colored their lineart for you or you have the characters old reference that has only a few inaccuracies etc. please post it. It will be a lot of help, but like I said if you have no art of the character you can just put "None".
All text you want on reference sheet: "Coffee Girl
Sex: Female
Sexuality: Unidentified
Personality: -Insert personality here-
Backstory: -Insert summary of backstory here-"
And near her eyes I would like it to say "Bags under eyes"
