Ah! Sorry, it did not, due to the fact that the image was broken at the time and I had no way of keeping an actual reference of your dog.
If you could, please resend your form with a bigger image of your dog so I can clearly see what it is I'm drawing.
If you don't have a bigger image on you, then please find an image similar to your dog that's clear with its markings so I can try my best to portray the dog accurately!
