I'm quoting this because it seems to have been missed. From here, any forms not following this will be ignored. The "new storage form" is purely for storages created AFTER you claim your first level. In case you move from Toyhouse to CS, or another storage site.

When you first create your storage and receive your founder, you'll want to head to Leveling instead and fill out the "Claiming My Founder" form! This has a section for your archive, and is the best way to get your first storage documented!
