How do you link an image with a url?

I've seen some people put so a thing in their sig where you click an image and it takes you to a site. I'm redoing mine, and I want to know how to do it! Can anyone help me? Thanks!

It doesn't seem to be working. I copied and pasted it and filled out the blanks. The image shows up, but the link appears beside it.
