The word "wanker" has multiple meanings. You are the one who chooses for it to be inappropriate, not the creator of the drawing. The term is used to mean "stupid" or "idiot" which is exactly what the character England means for it to be in the anime/manga Hetalia, including this drawing. I hope I'm not coming off as rude, but that is what it means to people these days. The original meaning of the insult is rarely ever used.
