I love the passion in the form, it really shone through as I read through it. My only critique would possibly be if you made the text this size (small). But that's personal preference I guess; to me it looks a little neater.
Maybe add some more information about Nemo in the top bit, where you are talking about him being your first kia etc.

Just put something like:
Name;; Nemo
Age;; 18 years, but occasionally acts more like eight!
Gender;; Male
_________________
then start your story ? you can use that if you want by the way xD
