I think you've misunderstood ^^ you dont have to use that form unless you've already created your MYO dragon and the dragon is approved by a staff member. Its basically saying that you've used a ticket and we need to update the bank. If you want to be added to the thread to create a MYO dragon the base please let us know and I'll add you! Just make sure you let a staff member like me or supernovacity know when you finish so we can approve it for you.
