Okay, well, i have made the poll. There are three options for now as this is what i could come up with.

There isn't a specific form, you have to create your own and pretty it up however you want. c:
Although, the forms have to include these: A character image, name, age, gender, personality, history, role.
Other than that, you may add whatever other information you want. :3
