Well in the official games, in the 4th gen (excluding the gen 2 remakes), gen 5 and gen 6 (as well as the 3rd gen remakes) there is a Moss Rock and an Ice Rock. The moss rock evolves eevee into Leafeon and the Ice rock into Glaceon if you level them up near it. I can't tell you specific areas right now, but you can check on bulbapedia.

Here I think there might be an item or two to replace them, but I'm not sure.

I'm not the owner now so I can't say for sure this is how it's gonna go, but when I was the owner you could receive the items "Ice Rock" and "Moss Rock" and those would evolve Eevee.

So I seen this, and I'm quite interested, though I'm sure like others I have questions. But is there any latest forms, sign ups. This seemed to go on for quite awhile so I was wondering if it had ever been updated and there was a newer form somewhere.

I highly daubt that there is. As far as I know it only changes once the regions switch due to the starters changing.
