General Computer/Firefox Help?

I hope this is alright for me to post here, I'm hoping someone here knows how to fix this. x.x
First off, I have a Mac OS X version 10.5.8 and I use Firefox 7.0.1.

So I read for the first time today about hard refreshing, and decided to give it a try. I held down the command key and hit the refresh button at the top of the page, and then this black box started showing up. This is what it looks like. It stays over the red circle when I'm here on Firefox, but when I quit FF, it moves to the first file on my desktop. It also seems to highlight whatever it is I have open, for example, when I open pictures in Preview.
Also, I noticed this, where the refresh, shut down, and log out choices were doubled up, which appears in a few other places now as well. I don't know why it's doing this, and I am wondering if someone could please help. Thanks so much! ^^
