New outcomes with items

I have seen new outcomes like


that have an item attached,
Where do they come from?, where do they go?, I don't think I have seen one on any of my pets nor do I see them in my items

I am sure it's something fairly obvious

They're posted that way by users in the monthly outcome threads-
viewtopic.php?f=20&t=589466

Its so the person in control of posting the different outcomes can get a clean transparent image to use without the user's name attached.
