Form Codes and Banners?

Firstly, how do I get forms on my adoptables page? I have a cupcake adoptable page and I need forms for it.

Secondly, how do I make a banner to put in my siggy? I want one so I can put links to my adoptables and my story (and possibly an artshop I might open).

Please help!

1. Forms are like this: [*code]Form here:[/code] but without the *. Then it should show like this:

2. Banners can be made on many websites, including Picnik.com, and also on programs such as GIMP which you can download to your computer. You upload them to Photobucket/ImageShack to have the codes.
